for Student Athletes
Planning on being an MCC athlete? Here are a few things you need to know prior to participation on an athletic team at MCC:
The National Junior College Athletic Association (NJCAA) requires that all first year athletes submit to your coach a completed Health History and Exam Form signed by your primary care provider 4 weeks prior to the start of your first practice.
Returning Athletes (2nd year) are required to complete a Returning Athlete's Medical History Questionnaire and have a signed clearance from their primary care provider. This form needs to be completed and turned in to your coach 4 weeks prior to the start of practice.
In addition to the Immunization Requirements of all students in the general student population, athletes must also have:
- Tetanus shot within the last ten (10) years
Insurance Requirements & Information
Student Athletes are required to have a comprehensive insurance plan while participating on an athletic team. That information including the ID number and a copy of both sides of your insurance card needs to be turned in with your physical or returning athlete forms and is a part of the clearance process. If you need to purchase insurance please review The Allen J. Flood Companies, Inc. brochure and complete their enrollment form. If you do have an injury please be sure to report it promptly to ensure you get the care needed in the quickest amount of time.
When filing an insurance claim please print the claim filing instructions and complete the insurance claim form and submit it to the insurance company directly. If you have any questions on filing a claim you can contact Klais & Co, Inc.. directly at: 1-800-331-1096. Once a claim has been filed and you would like to check the status please visit Klais & Co., Inc website and follow the prompts in the right corner under "STATUSLINK".
